What is a Patient Centered Medical Home
A Patient Centered Medical Home (PCMH) is health care focused on you, the patient. It is a partnership between you and your health care provider. Your provider is part of a healthcare professional team committed to improving your health and helping you reach health goals.
Who is my health care team?
You! Your health care team at Raphael Health Center also consists of your Medical Provider, and may include your Dentist, Behavioral Health Counselor, Optometrist, a Medical Assistant, and Front Desk Staff. Raphael Health Center also has a full-time Financial Navigator.
What are the advantages of a PCMH?
The team looks at your whole health. In addition to in-person appointments, twenty-four-hour access is available by phone and a patient portal. The team also coordinates care with specialists, hospitals, emergency rooms, and community services resources.
What does my health care team do?

You are an active partner in your care. You follow the agreed upon plan. You ask questions. You learn how to stay healthy.
What should I do when I come to an appointment?
Bring a list of your concerns, and medication and health changes since your last visit. Bring any forms that you need your provider to sign. Notify your team if you need medication refills, more information, or have questions.
What services does Raphael Health Center offer?
Medical, Dental, Behavioral Health including substance abuse and addiction treatment services, Optometry, Care Management, Financial Navigators, Lab Tests, Prenatal Care and HIV Prevention Care.

In Person: Evening hours are offered on Mondays and Wednesdays for most services.
Patient Portal: Ask our front desk staff how to sign up.
The patient portal is a new feature we offer our patients! It gives you access to request medication refills and appointments, update personal information, pay your bill online, and obtain basic information from your appointments. It’s a convenient time saver!
