Optometry

Raphael is pleased to partner with the Indiana University School of Optometry to provide primary optometric health care services that include preventive care and education, comprehensive vision examinations, intermediate vision examinations, eye-wear orders, and diagnosing, treating and managing diseases and disorders of the eye.

Services include:

  • Comprehensive Eye Examinations
  • Pediatric Vision Care
  • Eye Disease Diagnosis and Management
  • Pre- and Post- surgical Care
  • Eye Infection & Dry Eye Treatment
  • Management of necessary Eye and Medical Referrals
  • Frames and lenses

Optometry is part of Raphael’s Patient Centered Medical Home (PCMH). We encourage connection to and integration with other services, such as:

  • Specially trained staff to assist patients in applying for new federal health plans and other public benefit programs
  • Case management and patient navigation services to coordinate the medical, specialty, educational, and mental health needs of patients with chronic conditions
  • Pharmacy/Prescription assistance to provide low or no-cost medications
  • Foreign language and sign language interpreters
