In addition to these health services, our patient-centered care includes:

  • Specially trained staff to assist patients in applying for new federal health plans and other public benefit programs.
  • Care management and patient navigation services to coordinate the medical, specialty, educational, and mental health needs of patients with chronic conditions
  • Family planning, annual gynecological and breast exams, screening and treatment of sexually transmitted diseases and HIV, and pregnancy testing
  • Pharmacy/Prescription assistance to provide low or no-cost medications
  • Foreign language and sign language interpreters

Every Raphael Health Center medical patient works with their primary care team, which may include family medicine physicians, nurse practitioners, physician assistants, behavioral health nurses, and medical assistants.

It is important our community members know that Raphael is committed to serving their Behavioral Health needs.

No one will be denied care based on their inability to pay at Raphael Health Center.
