Scheduling
Our phone lines are open from 7:30AM to 5:00PM, Monday-Friday. Same day appointments are available, as scheduling allows.
When scheduling your first appointment, be prepared to give the following information over the phone:
Name, Address, and Contact Information
Date of Birth
Social Security Number
Health Insurance or Healthcare Assistance Information, if applicable
Your First Appointment
New patients will need to complete their registration information either before your visit using your own device or when you arrive using a Phreesia Pad. Arriving 20 minutes early to your first appointment allows time to complete the new patient intake and health history information.
Please remember you will need to provide the following at your appointment:
Up-to-date email address or phone number
State Issued Photo ID
Proof of Insurance (card or letter)
Co-Payment (if required by your insurance)
List of medications, both prescribed and over the counter, plus any vitamins, herbs, and supplements
Insurance and Sliding Fee Scale
No one will be denied care based on their inability to pay.
Raphael Health Center accepts Medicaid, Medicare, and many types of commercial insurance. In addition to these, we operate the Federal Sliding Fee Discount Program. This program allows you to receive medical, behavioral, optometry, and dental health services at a discounted rate, based on your family size and income.
If you qualify for the Federal Sliding Fee Discount Program and have private insurance, Medicaid, or Medicare, you still receive the full benefit of the Sliding Fee. After insurance/Medicaid/Medicare are billed for your care, any remaining balance is billed at your qualifying tier on the Sliding Fee Scale.
To take advantage of the Federal Sliding Fee Discount Program, patients must meet with a Financial Navigator at Raphael Health Center to enroll. To schedule an appointment with a Financial Navigator, call (317)-926-1507, Monday-Friday, between the hours of 7:30AM and 5:00PM.
During your appointment with a Financial Navigator, Raphael Health Center staff will provide information on eligibility for Medicaid, Marketplace, HIP Enrollment, and the Federal Sliding Fee Discount Program, as well as assist you in enrolling for programs for which you qualify.
Documents needed to determine eligibility are:
State Issued Photo ID
Social Security Card
Birth Certificates (for dependents age 19 and under)
Proof of Income
Any ONE of the following documents may be submitted as proof of income:
- Pay stubs for the last 90 days
- Child Support Award Letter, if only income
- Proof of Unemployment (WORKONE)
- Social Security/SS Award Letter
- Disability Award Letter
- Workman’s Compensation
- Previous Year’s Completed Income Tax Return or W-2
